No milligram of Viagra exists beside a nitrate
Titration talk is how men try to outrun a contraindication. They will offer to 'just use 25 mg' next to a nitrate spray. That sentence is a refusal on this chart. Dual cyclic-GMP elevation does not respect a smaller diamond.
Riociguat is the same refusal. Hypersensitivity to sildenafil or a tablet component is the third. I will not fine-tune milligrams until those boxes are empty. Strength is a later row.
If angina care depends on a nitric-oxide donor, we stop talking about 25 versus 50 and start talking about non-PDE5 options with cardiology. The sildenafil vital will not sell you a loophole strength.
Why 25 mg is the start, not the Viagra ceiling
People hear 25 mg and think starter candy. The label uses it as the decrease from 50 mg when effects or other drugs demand less exposure, and as the considered start when clearance is slow. It is a working dose. It is not the top of the ladder.
The ceiling for as-needed ED tablets is 100 mg once daily. Climbing past that in healthy volunteer stories (200 to 800 mg) produced hypotension, syncope, and prolonged erection. That is not a target. That is a warning about leftover internet folklore.
A prescription that begins at 25 mg is often the safer arithmetic, not a slight. If several aroused, well-timed nights at 25 mg are tolerated and still weak, 50 mg is the usual next stamp - written, not improvised as two tablets. Price talk does not change the ladder. A cheap 25 mg fill is still this sequence.
Partners sometimes treat 25 mg as a negotiation. I do not. The opening stamp is exposure math: age, liver, kidney, CYP3A4, alpha-blockers. Desire for a dramatic onset is not a kinetic variable. I will explain that in the room once. I will not raise a milligram to end an argument.
The labeled ladder is 25, 50, and 100
Film-coated tablets: 25, 50, 100 mg. For most patients the recommended as-needed dose is 50 mg about an hour before activity, anywhere from 30 minutes to 4 hours. Based on effect and toleration, the prescriber may raise to 100 mg or drop to 25 mg. Frequency cap: once per day.
This site's SERP lock is 25 mg because that is the strength this domain chose to keep in the title row. The body still has to tell the whole ladder. Do not read our 25 mg lock as a claim that 50 mg vanished from the US label. It did not.
Pulmonary arterial hypertension uses a different sildenafil chart - smaller milligrams on a three-times-daily schedule under a different brand history. Do not borrow a Revatio-style rhythm for an ED diamond. Wrong indication, wrong arithmetic.
Kinetics stay dose-proportional across the ED range. Doubling the tablet roughly doubles the area if clearance is unchanged. That is why 25 to 50 is a real step and why two 25s in one night is not a clever half-measure. It is 50 mg with a frequency lie.
N-desmethyl metabolite keeps about half the parent PDE5 potency in vitro and about 40 percent of parent plasma levels, so it adds roughly a fifth of the effect. I do not dose that metabolite separately. I just refuse to pretend the curve is gone at hour three.
Who the label keeps on 25 mg
Consider 25 mg as the start if age is over 65, if the liver is impaired, or if creatinine clearance is under 30 mL/min. Elderly volunteers showed about 84 percent higher parent AUC and 45 percent higher free-drug AUC versus young adults. Severe renal impairment roughly doubled Cmax and AUC. Hepatic impairment in Child-Pugh A and B raised Cmax about 47 percent and AUC about 85 percent. Child-Pugh C was not studied.
Strong and moderate CYP3A4 inhibitors raise the curve. Erythromycin: Cmax up about 160 percent, AUC about 182 percent. Saquinavir: Cmax about 140 percent, AUC about 210 percent. Ketoconazole and itraconazole are expected to hit at least as hard. The labeled move is to consider 25 mg as the start.
CYP3A4 inducers such as rifampin can make a 25 mg tablet feel thin. That is a prescriber problem, not a same-night double. Bring the full list. Do not hide the antifungal.
Loop and potassium-sparing diuretics raised the metabolite AUC about 62 percent in trial PK notes. Nonspecific beta-blockers raised it about 102 percent. The label did not treat those metabolite bumps as a reason to rewrite the ED ladder. I still want them on the list when a 25 mg night feels louder than expected.
Warfarin, SSRIs, thiazides, ACE inhibitors, and calcium-channel blockers did not move sildenafil PK in those same notes. They can still move blood pressure. A clean CYP story is not a clean pressure story.
Ritonavir turns 25 mg into a 48-hour ceiling
Ritonavir boosted sildenafil AUC about eleven-fold. The labeled cap is a single 25 mg dose and no more than 25 mg in 48 hours. That is not a suggestion for bargain hunters. That is an exposure clamp.
Healthy-volunteer high-dose stories in that section of the label - 200 to 800 mg - mentioned decreased blood pressure, syncope, and prolonged erection. I read those as a reason to respect the ritonavir cap, not as a curiosity.
If HIV therapy includes ritonavir, the ED conversation happens with both prescribers in the same week. A cheap 25 mg bottle bought against that cap is how prolonged erection and a faint share a night.
Saquinavir is a strong CYP3A4 inhibitor with a milder published bump than ritonavir and still earns a 25 mg start. I do not let a man treat 'not ritonavir' as permission to open at 100 mg while an azole or a macrolide is running.
Alpha-blockers want the same low stamp
Both classes vasodilate. Additive drops show up as dizziness, lightheadedness, or a faint. The man should be hemodynamically stable on the alpha-blocker first. Then sildenafil starts at 25 mg. If he is already optimized on sildenafil and an alpha-blocker is added later, the alpha-blocker starts low.
Volume depletion and extra antihypertensives make the stack worse. I separate peak times when I can. I do not let a man 'see how 50 mg feels' on the first shared week.
This is coordination, not self-titration. The side-effects vital is where the faint gets named. This page is where the opening milligram gets chosen.
Two tablets in one day is not titration
Two 25 mg tablets is 50 mg plus a lie about frequency if they share a 24-hour block after a 'weak' first swallow. The labeled once-daily cap exists because leftover occupancy and metabolite are still on the curve.
True titration is a later date, a written strength, and a fair trial at the old strength with a light plate and actual arousal. See timing and food before you blame 25 mg for a steak.
Splitting a 50 mg tablet to improvise 25 mg is a pharmacy-and-prescriber question, not a kitchen craft. Some film-coats are not meant for amateur geometry. Ask. Do not guess.
Scored tablets still deserve a pharmacist's yes. Uneven halves are not a validated 25 mg start. I would rather write 25 mg on the script than watch a man eyeball a blue diamond over the sink.
When 25 mg looks weak and when it is just early
Early: heavy plate, no arousal, first-night panic, a pour that put the central signal to sleep. Those nights do not earn 100 mg.
Weak after a fair trial: several attempts, light food, hold box empty, 25 mg tolerated. Then 50 mg is the usual written step. If 50 mg works but the headache is ugly, coming back to 25 mg is a success, not a demotion.
The mechanism still needs nitric oxide. A bigger tablet cannot invent it. If 100 mg is tolerated and still blank, stop climbing and look at vessels, nerves, hormones, and the hold-not-start story. FDA's older full label is a public copy of the same ladder: FDA Viagra label (PDF).
What I write first on a 25 mg chart
Hold box empty. Indication is ED, not a borrowed PAH schedule. Opening strength 25 mg when age, liver, severe kidney, alpha-blocker, or a CYP3A4 inhibitor argues for it. Otherwise the label's 50 mg start is the usual community default - and this domain still teaches 25 mg as the careful first stamp many of my own patients actually get.
One swallow per 24 hours. Ritonavir: 25 mg, 48-hour ceiling. Review after several clean nights, not after one restaurant.
Your clinician writes the actual prescription. I sign teaching vitals. I do not ship tablets.
Bring the interacting list to that visit: azoles, macrolides, protease inhibitors, alpha-blockers, nitrates, riociguat. A missing row is how a careful 25 mg start becomes an accidental 50 mg night with leftover occupancy. Write the list before you sit down.
Dose mail is exposure mail. I want the hold list, the interacting drugs, and whether two tablets already shared a night before I talk about 50 mg.

Two 25s tonight is a same-day stack, not a titration. It violates the once-daily cap and adds vasodilation without a cleaner peak.
If 25 mg was tolerated and the nights were fair - light plate, arousal present - 50 mg on a later date is the usual written step. 100 mg is a later step still, not a rebound from one blank evening.

You are exactly who the 25 mg start was written for. Age raises AUC. Severe renal impairment can double it. The same tablet is a higher exposure in your plasma than in a 35-year-old's.
We step up only if 25 mg is safe and still weak after a fair trial. Pride is a bad reason to open at 50 mg.
Quentin H., 53, asks: I am on an azole antifungal and tamsulosin. Does that lock me at 25 mg?
It argues hard for 25 mg as the start. Azoles inhibit CYP3A4 and raise sildenafil AUC. Tamsulosin adds hypotension risk. Both point to the low stamp and separated peaks.

Because this domain locked 25 mg in the title row and because I often open there when the chart is not a healthy 40-year-old with an empty medication list. The US label's 'most patients' line is still 50 mg.
Both sentences can be true. Read the whole ladder. Do not treat our SERP lock as a rewritten FDA monograph.

The label is right: 25 mg, and no more than 25 mg in 48 hours. Eleven-fold AUC is not forum folklore.
Weekly frequency is a separate preference about how often you want a window. It does not relax the 48-hour cap when ritonavir is on board. Do not invent a Saturday double because Friday felt polite.

Yes. Try 50 mg on a clean plate. Many men keep the effect and lose the worst Cmax headache. Lowest effective dose is the goal, not a trophy milligram.
One hundred milligrams is the ED ceiling. Higher is not a clever move. If 50 mg fails and 100 mg is the only tolerable efficacy, we talk about whether the headache is worth it - out loud, not as a silent martyrdom.
Cyrus W., 31, asks: Can I use leftover pulmonary sildenafil tablets as a cheap 25 mg ED dose?
Do not remix indications. PAH regimens are a different schedule and a different supervision. Cutting those tablets into a weekend ED plan is how people scramble both charts.
If you have leftover PAH sildenafil, that belongs to the prescriber who owns the lungs. Ask for an ED plan on paper if that is the new goal.
Odette's partner Glen, 57, asks: Is a cheap generic 25 mg prescription weaker than brand Viagra 25 mg?
A licensed generic has to meet bioequivalence. The enzyme does not know the logo. Counterfeit '25 mg' from an unlabeled seller is the weak or dangerous one.
Pay for a real prescription path. Do not treat a mystery tablet as a dose-finding tool.

Several aroused attempts with a light plate and no second tablet. I will not pick a magic integer for every man. I will not judge a single restaurant either.
If those nights are blank and the hold list is clean, we write 50 mg or we look for another cause. We do not keep collecting 25 mg blanks as a personality test.
